Create Expiration Item - Missing Field

Hello, this is my feedback on missing fields from the “Create Expiration Item” form since the new ER version. I am missing a field I created for “Comments” that is generated in my custom reports when sharing with other stakeholders of my organization. I am having to add the comments after creating a new Expiration and do not see anywhere that I can add the “Comment” field back to the form. Is it possible to add my custom field back to the intake form permanently?
